Student-athletes must have these forms completed and turned in before they can be eligible to participate:
1. Parent Permit (signed by parent and includes health insurance information for student)
2. Pre-participation physical form (signed and stamped by a physician)
Once forms are completed they can be turned in 2 ways:
1. Papers are dropped off at the school in the drop box out front
OR
2. Papers are scanned and emailed to [email protected] for processing.
Athletes are also required to maintain a 2.0 Grade Point Average (GPA)
